Here is also the sample file how to fill it in for those authors whose papers have been accepted as IEEE publications.

Electronic copyright form should also be submitted electronically through EDAS before May 2, 2017.  The file-name of the copyright form should include the number of submission in the EDAS system and section number (for example, IFOST_2017_CRT_1570336005_Sect1.pdf).
